One of Lil Wayne's bodyguards has reportedly had a police report filed against him by a 15 year-old boy who claims he was assaulted during an altercation last week in Arizona.

The incident reportedly took place in Phoenix after Weezy's camp was leaving a concert and stopped near a residential neighborhood.

It all went down after Wayne's show last Wednesday (August 12) at the Cricket Pavilion in suburban Phoenix -- when 2 buses from his 4-bus entourage stopped at a nearby apartment complex. Law enforcement sources tell us witnesses saw several women getting on and off the buses -- and it was then that certain people in the complex began to get violent. Witnesses told police they heard at least one shot fired during the altercation -- though it's unclear who fired it. (TMZ)

An unnamed teenager reportedly contacted police authorities shortly after the altercation.

Cops arrived to the scene and broke things up -- but we've learned a 15-year-old boy has since filed a police report claiming one of Wayne's bodyguards roughed him up during the chaos, bruising a few of his ribs. Cops interviewed Wayne at a nearby hotel after the incident, where the rapper told cops he was on one of the buses when the attack started -- but his bus took off almost immediately. (Singers Room)

The rapper later canceled his Canadian shows this week due to an "illness."

"Lil Wayne, due to illness, was advised by his doctors to cancel the Young Money Presents: America's Most Wanted Music Festival tour stops in Western Canada," a statement to SOHH read from the rapper's camp. "The tour, headlined by Lil Wayne featuring Young Jeezy, Soulja Boy, Pleasure P and Jeremih, was scheduled to come through Vancouver and Edmonton on August 17th and 18th. Wayne, his band and crew arrived in Canada with full intention of performing but were forced to cancel the dates due to doctor-mandated rest. Young Money Presents America's Most Wanted Music Festival will continue their tour routing as planned and extends an apology to the fans for the cancellation." (Statement)

News of Tuesday and Wednesday's cancellations landed online earlier this week.

Tonight's Lil Wayne concert at GM Place has been canceled. "The Lil Wayne show has been canceled due to circumstances beyond Live Nation's control," the promoter said in a news release. The rapper last performed in Vancouver in January. Lil Wayne, who dominated rap music at the 2009 Grammys winning four awards, was to share the bill with Young Jeezy, Soulja Boy and Drake.
